Community Counseling of Bristol County (CCBC) is seeking an entry-level Case Manager in Brockton, MA, for the PACT-Y program. You will work within a multi-disciplinary team to support youth transitioning to adulthood, providing community-based services, job exploration and access to resources.
Responsibilities include coordinating care with families and DMH, documenting per guidelines and offering on-call/weekend support. A Bachelor’s degree and driving means are required.
