Countryside Care Center is looking for a compassionate and motivated Speech-Language Pathologist (SLP) to join our rehabilitation team. You'll play a key role in helping residents regain communication, cognitive, and swallowing abilities while working in a collaborative skilled nursing and post-acute care environment.

• Perform comprehensive speech, language, cognitive, and swallowing evaluations
• Design individualized treatment plans that promote functional independence
• Deliver evidence-based therapy for speech, communication, cognition, and dysphagia
• Monitor resident progress and update treatment plans as needed
• Collaborate with physicians, nursing staff, PTs, OTs, and families to provide coordinated care
• Maintain timely, accurate documentation in compliance with regulatory requirements

• Treat a diverse caseload of short-term rehabilitation and long-term care residents
• Work alongside an experienced interdisciplinary therapy team

• Active Illinois Speech-Language Pathologist license required
• Excellent communication and clinical decision-making skills
• Experience in skilled nursing or rehabilitation is preferred, but not required
