RN MDS Coordinator

Countryside Care Center and Macomb Post Acute Care Center are seeking an RN MDS Coordinator to support both skilled nursing facilities. The buildings are conveniently located less than one mile apart.

The RN MDS Coordinator is responsible for coordinating, scheduling, and completing MDS assessments while ensuring accurate clinical documentation and appropriate reimbursement.

• Active Illinois RN license required
• MDS experience preferred
• Thorough understanding of the MDS process, including coordination, scheduling, and completion
• Skilled nursing facility or long‑term care experience required
• Strong organizational and communication skills

• $90,000 annual salary
