- Sign-On Bonus: $10000
- Compensation:
Base salary up to $235,000 + $25,000 of potential bonuses based on key performance indicators, not limited to productivity, patient satisfaction, and quality
- Signing bonus up to $10,000
- Relocation assistance up to $10,000

PTO Accrual + 9 Paid Holidays
• Years 1-2: 120 hours
• Years 3-5: 160 hours
• Years 6+: 200 hoursCME
• 40 Hours per year of paid CME benefit
• Paid Registration and Travel, up to $3000
Retirement
• 403b Match: 3% for years 1-5, 5% after 5 years
License Maintenance
• State License Annual Renewal
• DEA - 3-year Renewal
• FTCA - Professional Liability Coverage Phone Allowance - $1,200
