Primary Care Setting:
Outpatient Health Center Seeking a Board Certified or Board Eligible Primary Care Physician to provide comprehensive medical care in a community health center setting in Macon, Georgia. This position focuses on preventive care, chronic disease management, and patient-centered treatment within a collaborative clinical environment.
Position Highlights:
Provide full-spectrum primary care services including diagnosis and treatment of acute and chronic conditions
Deliver preventive care focused on health risk reduction and wellness promotion
Practice consistent with evidence-based medicine and organizational clinical policies
Work within a Patient-Centered Medical Home model
Collaborate with multidisciplinary healthcare teams
Participate in quality assurance and performance improvement initiatives
Opportunities to contribute to medical policy development and clinical leadership activities
Serve diverse patient populations in a community-focused setting
Responsibilities:
Perform comprehensive histories and physical examinations for new and established patients
Order and interpret diagnostic testing and coordinate specialist referrals
Develop treatment plans and manage medication prescribing and refills
Arrange hospital admissions, discharges, and transitions of care Maintain accurate and timely electronic health record documentation
Participate in staff meetings and peer review activities
Support quality improvement and meaningful use initiatives
Provide patient education and promote preventive health practices
Qualifications:
Board Certified or Board Eligible in a Primary Care specialty
Active Georgia medical license or eligibility
Experience in community or public health settings preferred
Strong clinical, communication, and interpersonal skills
Ability to work independently and collaboratively in a team-based environment
Knowledge of clinical operations, quality standards, and documentation requirements
