Registry with the Commission on Dietetic Registration with the Academy of Nutrition and Dietetics and License issued by the Georgia Board of Examiners of Licensed Dietitians (RD/LD).
Two years of clinical experience, preferably in diabetes patient education and supervision.
Physical Requirements And Working ConditionsMust be able to use visual acuity to see monitor screen, computer and hard copy materials. Must be able to hear and verbally communicate in person and over the phone or radio. Must be able to sit for prolonged periods of time (up to 2 hours). Must be able to comprehend and learn operation of various office equipment. Must have functional range of motion of the cervical, thoracic and lumbar spines, upper and lower extremities with a grip strength of 50-60# specific to job evaluation.
Must be able to forward reach, overhead reach, bend, squat, kneel and apply proper body mechanics during the transfers and transport supplies and/or equipment using proper body mechanics. Must be able to lift up to 10# specific to job evaluation.

Coordinates the clinical teaching/learning aspect of the diabetes program for Diabetes Health ways (DH). Activities include patient and family assessment and counseling regarding diabetes and its management, as well as teaching on an individual or group basis. May teach all components of the DH program as needed, including nutrition and medication management once cross trained. Acts as a case manager for assigned patients with diabetes, assessing patient and family needs regarding diabetes management and education, and coordinates their treatment plan with physicians and other treatment team members.
Actively participates in support group activities, taking leadership roles when appropriate. Effectively manages inquiries. Screens visitors for diabetes as needed, taking appropriate action based on results. Actively participates in marketing the center program to physicians, nurses, patients and organizations. Provides liaison with nursing staff. Represents the DH as a member of various committees as assigned. Attends weekly patient care conferences, presenting cases as appropriate.
