Job Responsibilities

Responsible for providing comprehensive HIV Case Management services, including support, care coordination, and treatment services for individuals living with HIV/AIDS and their families. Duties include conducting client assessments, coordinating medical referrals, and facilitating access to social support services. As an integral member of the Infectious Disease Program team, this position performs expanded functions requiring specialized clinical knowledge, critical thinking, and sound judgment in the delivery of client‑centered care.

The Case Manager develops and implements individualized plans of care based on comprehensive assessments to ensure continuity of services and optimal health outcomes. This role also assists clients with medication access by facilitating medication assistance applications, co‑pay card enrollment, and patient assistance programs to ensure timely and uninterrupted access to treatment. The position operates in a fast‑paced clinical environment requiring strong organizational skills, effective multitasking, and excellent time management.

Additional responsibilities include providing nursing support and laboratory coverage as needed.

Minimum Qualifications

Associate’s degree in nursing from an accredited college or university, completion of a PHN course within one year of hire, current Georgia Registered Professional Nurse license, and CPR certification.

Preferred Qualifications

Completion of a bachelor’s degree in nursing, one year of experience as a registered professional nurse, and experience working with HIV/AIDS patients.

The Southeast Health District (SEHD) is committed to fostering a culture of health and well‑being among its employees. Employees may utilize up to 30 minutes per workday for wellness activities, which can be taken as two 15‑minute breaks or one 30‑minute break.
