Part-Time Home Health RN working with individuals with developmental disabilities.
20 to 30 hours per week.
DUTIES:
- Provide weekly assessments of individuals served
- Conduct supervisory visits and document results
- Annual oversight of health, risk and safety assessments using the Health Risk Screening Tool (HRST)
- Review of data tracking and documentation
- Staff training and oversight including proxy care
- Coordinate medical visits for clients
- Medication reconciliations
- Other duties as assigned
