This is a full‑time, permanent primary care role serving patients ages 2 and up within a collaborative, fully integrated care setting.

You will have the opportunity to build lasting relationships with your patients, practice comprehensive preventive care, and be part of a team that is genuinely changing lives in Central Georgia.

• Provide primary care across the lifespan — pediatrics, adults, and geriatrics
• Diagnose and manage both acute and chronic conditions
• Lead preventive care, patient education, and chronic disease management
• Collaborate with an integrated team including physicians, behavioral health, and dental providers
• Deliver culturally competent, compassionate care to an underserved and diverse patient population

• Board Certified Family Nurse Practitioner (FNP-BC or FNP-C)
• Active Georgia license or ability to obtain prior to start
• Primary care experience preferred
• Genuine passion for serving underserved and diverse populations
