We are seeking an experienced Nurse Practitioner to join our primary care practice. This full-time position provides the opportunity to care for patients ages 14 and older in a fast‑paced, professional environment. The ideal candidate is confident evaluating, diagnosing, and treating a wide range of acute and chronic conditions while delivering high‑quality patient care. We are looking for a provider who values patient relationships, communication, and teamwork and is passionate about helping patients achieve better health and quality of life.
Responsibilities- Provide comprehensive primary care services, including independent evaluation, diagnosis, and treatment of patients ages 14 and older.
- Conduct preventive, wellness, chronic care, and acute sick visit assessments.
- Develop, implement, and manage individualized treatment plans based on evidence‑based clinical practices.
- Order, review, and interpret laboratory tests, imaging studies, and other diagnostic procedures.
- Communicate test results, treatment recommendations, and follow‑up plans clearly and compassionately to patients.
- Educate patients and families on medical conditions, preventive care, medications, and healthy lifestyle practices.
- Coordinate patient care by initiating referrals to specialists and other healthcare resources when appropriate.
- Maintain accurate, thorough, and timely documentation within the electronic health record (EHR) system.
- Prescribe medications and therapies in accordance with state regulations and organizational policies.
- Monitor and manage chronic conditions, ensuring continuity of care and positive patient outcomes.
- Collaborate effectively with physicians, advanced practice providers, nurses, medical assistants, and administrative staff to deliver high‑quality, patient‑centered care.
- Support quality improvement initiatives and contribute to efforts that improve clinical outcomes, patient satisfaction, and practice efficiency.
