PrimaryDuties & Responsibilities
- Focuses on the prevention of illness, injury, or disability, the promotion of health and maintenance of the health of populations.
- Cares for populations with special needs during disasters/emergencies and biological and chemical protocols.
- Provides home visiting services to identify, assess and provide care coordination services to eligible perinatal patients and qualifying infants.
- Coordinates program services with other service providers and community organizations to promote optimal implementation.
- Screen and refer clients to other community service providers.
- Monitors participants according to program guidelines and provides education and materials that promote wellness, safety, and awareness of pregnancy and postpartum complications.
- Monitors infant complications, including feeding, weight, head circumference, and developmental screening.
- Develops a plan of care for individuals and families, modifying treatment plans as indicated by client responses and conditions.
- Provides referral/linkage when medical and environmental risks are identified to appropriate services.
- Promotes the program through community engagement and outreach events.
- Develops and sustains community partnerships.
- Delivers case management and nursing care according to public health protocols.
- Performs physical assessments, takes vital signs, performs fetal monitoring, and conducts basic lab screenings.
- Provides breastfeeding education and counseling and refers clients to appropriate social and health services.
- Maintains accurate records of patient care, services, and responses to treatment.
- Performs other duties as assigned.
This position is based at the North Central Health District Office in Macon, GA and requires travel across several counties within the district. Candidates should be willing to travel to locations throughout our district as needed. Employees in this role are eligible to receive mileage compensation for work‑related travel.
Required Qualifications- Associate's degree in nursing from an accredited college or university.
- Completion of a PHN course within one year of hire.
- Current Georgia Registered Professional Nurse license and CPR certification.
- Valid driver's license and reliable transportation.
- Bilingual in Spanish and English.
- Bachelor’s degree in Nursing (BSN).
- Experience providing interventions for high‑risk prenatal patients.
- Experience with high‑risk newborns and infants.
- Home visitation experience.
- At least one year of labor and delivery experience.
- At least one year of experience working in a prenatal care clinic.
- At least one year of experience working with mothers and infants.
