LocationCarlyle Place, Atrium Health Navicent is an inviting not‑for‑profit Life‑Plan community located in Northwest Macon. The facility includes 226 independent living units, 29 assisted living units, 26 memory care units, and 40 skilled nursing units.
Job SummaryPerforms nursing care and assists with planning and implementing care for the comfort and well‑being of the patient/resident. LPNs work under the supervision, orders, or directions of a Registered Nurse (RN), physician, dentist, or other licensed independent practitioner.
Essential Functions- Participate in determining nursing care needs based on collection and interpretation of relevant data.
- Recognize and respond to immediate intervention needs.
- Document patient/resident observations, treatments, and responses, and participate in treatment and procedures. Assist with discharge planning.
- Assist RN in developing/revising the plan of care by providing data and identifying goals and interventions for review.
- Communicate with appropriate clinical staff when significant changes occur.
- Implement the identified plan under supervision, ensuring tasks are completed.
- Monitor progress and report to the RN.
- Organize workload and prioritize responsibilities to ensure efficient, quality care.
- Maintain knowledge and competence within nursing practice and legal boundaries.
- Document assessments, procedures, education, and communication with the healthcare team.
- Provide and assess the effectiveness of patient/resident and family education within scope of practice.
Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be able to lift up to 35 pounds. Must speak English, have intact vision, hearing, smell, touch, finger dexterity, and critical thinking. Physical abilities testing required.
Education, Experience and CertificationsGraduate from an accredited School of Practical Nursing. Hold a current GA LPN license (or applicable national compact or temporary license). CPR/first aid required upon hire or within 60 days. BLS required per policy.
Benefits and CompensationPay Range: $25.30 – $37.95 per hour (based on qualifications, skills, experience, and training).
