We are seeking an experienced, transformational Director of Nursing (DON) to lead clinical operations for the Hospice program of our rapidly growing Houston, Texas-based home‑based care organization.
The Director of Nursing is responsible for ensuring exceptional patient and family‑centered end‑of‑life care, clinical quality, regulatory compliance, and professional practice across the Hospice service line. This leader will build and standardize clinical systems, develop nursing leadership, strengthen quality outcomes, and create a culture of clinical excellence that supports significant organizational growth.
The DON will partner closely with the COO, CFO, Chief Growth Officer, and executive leadership to ensure Hospice clinical operations remain patient‑centered, compliant, scalable, and aligned with the Company's long‑term strategic goals.
Clinical Leadership- Provide executive oversight of all Hospice clinical operations.
- Ensure delivery of safe, high‑quality, patient‑ and family‑centered end‑of‑life care.
- Develop and mentor Clinical Managers, Nursing Supervisors, and field clinicians.
- Promote a culture of accountability, professionalism, and clinical excellence.
- Standardize clinical workflows, documentation, and hospice care delivery processes.
- Oversee patient admissions, care planning, supervisory visits, rectification, and discharge/bereavement planning.
- Improve clinical productivity while maintaining exceptional quality outcomes.
- Partner with operations to improve referral‑to‑admission turnaround times and continuity of care.
- Lead the Company's Quality Assurance and Performance Improvement (QAPI) program for Hospice.
- Ensure compliance with all federal, state, Medicaid, hospice licensing, accreditation, and regulatory requirements (including hospice Conditions of Participation).
- Monitor clinical documentation, audits, incident reporting, infection prevention, and risk management.
- Lead corrective action plans and continuous quality improvement initiatives.
- Develop orientation, competency validation, and continuing education programs specific to hospice and palliative care.
- Build leadership development programs for nursing supervisors and clinical managers.
- Partner with Human Resources to improve nurse recruitment, onboarding, retention, and engagement.
- Foster a positive clinical culture that attracts and retains exceptional hospice nursing professionals.
- Develop and monitor clinical dashboards tracking patient outcomes, hospitalizations/readmission, infection rates, documentation quality, hospice quality measures, clinical productivity, patient/family satisfaction, staff competency/retention, and survey readiness.
- Use clinical data to drive continuous improvement and support operational decision‑making.
- Active Texas Registered Nurse (RN) license in good standing
- Bachelor's degree in Nursing (BSN) required;
Master's degree preferred - Minimum of 5 years of nursing leadership experience in relevant home‑based care settings
- Strong knowledge of Medicaid regulations, clinical operations, and quality improvement
- Proven ability to lead teams, develop leaders, and improve clinical performance
- Experience in Hospice or palliative care strongly preferred
- Experience with hospice survey readiness, accreditation, and regulatory compliance
- Strong knowledge of electronic health records and healthcare technology
