Nurse Practitioner

We are seeking a dedicated and compassionate Family Nurse Practitioner to join our collaborative clinical care team. In this role, you will manage a dedicated patient panel, providing comprehensive, evidence-based primary care. You will work autonomously while being supported by a cohesive team of healthcare professionals to drive positive health outcomes. • Provide comprehensive primary and family care, including accurate assessment, diagnosis, treatment, and management of acute and chronic health problems.
• Perform routine examinations, health counseling, and disposition planning to ensure positive patient health outcomes.
• Coordinate with other healthcare professionals to maintain a comprehensive and integrated plan of care.
• Encourage patients to become active participants in their own healthcare through health screenings, behavioral health support, wellness education, and chronic condition management.
• Utilize diagnostic tests and interpret results to effectively diagnose and treat injuries or illnesses.

• Manage patient appointments, follow-ups, and routine examinations efficiently.
• Handle administrative tasks including verifying insurance, collecting payments, and patient follow-up communication.
• Assist with clinic opening/closing procedures, inventory management, and maintaining a clean, safe environment.

• Master’s Degree (or higher) from an accredited Family Nurse Practitioner program.
• Current National Board Certification (e.g., AANP or ANCC) as a Family Nurse Practitioner.
• Active State of Employment License to practice in the Advanced Practice Nurse role (willingness to obtain additional multi-state licensures is required).

• Minimum of 1 year of recent Primary Care experience in a family medicine, primary care practice, or urgent care setting (required).
• Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
