Registered Nurse (RN) - Hospital at Home Days​/Nights PT

Schedule

48 hours per pay period. Days: 7:00a-7:30p. Two 12‑hour shifts per week. Nights 7:00p-7:30a. Two 12‑hour shifts per week. Weekends:
Every other weekend, rotating Saturday/Sunday. Holidays:
Rotating Holiday Groups.

$38.20 - $57.30

Position based out of Macon, GA (Hybrid role).

• Completion of an accredited or approved nursing program.
• Registered Nurse license issued by the state in which you practice.
• Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association.
• 2 years of recent inpatient experience, preferably in ER or ICU settings.
• GA RN license.
• Strong communication and technology skills for virtual care delivery.

• Provide 24/7 oversight of patients receiving acute‑level care at home, completing admission and discharge workflows via Epic, including patient education and care plan initiation.
• Communicate urgent findings via phone and routine updates through Secure Chat.
• Act as a skilled and compassionate RN, coordinating comprehensive patient care, delivering safe therapeutic care per standards, policies, and procedures.
• Use evidence‑based practice and clinical decision‑making while fostering a safe patient environment and best experience across the continuum.
