Heartland Hospice is seeking an After hours RN Care Manager to provide high-quality hospice care outside standard hours. You will ensure continuity of care, provide direct after-hours visits, and maintain clinical oversight for assigned patients.
As a senior on-call clinician, you will collaborate with physicians and the IDG, document thoroughly, and lead field staff during nights, weekends, and on-call periods. A valid RN license and 1 year hospice/home health experience are required.
