Care Manager

Piedmont Healthcare is seeking a Care Manager to coordinate patient care across the continuum in a collaborative, patient-centered environment. In this daytime role, you will assess patient needs, develop and implement care plans, and partner with physicians, nurses, and social workers to support safe transitions from hospital to home or other care settings. You will educate patients and families, align services with community resources, and document in the EHR.

Responsibilities

• Coordinate and manage patient care plans across multidisciplinary teams
• Conduct patient assessments to identify medical, social, and support needs
• Collaborate with physicians, nurses, and social workers to ensure seamless transitions of care
• Educate patients and families on care plans, resources, and self-management
• Monitor patient progress and adjust care plans based on outcomes and feedback
• Facilitate discharge planning and connect patients with community resources
• Document all care coordination activities in the electronic health record
• Advocate for patient needs while supporting organizational quality and safety goals
• Participate in quality improvement initiatives and care management programs
• Maintain compliance with healthcare regulations, policies, and standards
