36 hour employee 0.9 FTE Salary position paid 100% by the DPH contract funding through maternal outreach.

Pay Range

$38.20 - $57.30

Major Responsibilities

Grant Coordinator Specifics:

  • Tracking of all obstetrical patients on a daily, monthly basis
  • Establish patient’s high risk criteria, verify medical eligibility.
  • Provide the grant applications to potential applicants and explain the purpose of the grant funds as identified by Grant criteria
  • Follow up and track the completion of the applications.
  • Verify financial eligibility in accordance to the Federal Poverty Level Guidelines.
  • Maintain grant files.
  • Responsible for approving allocation of grant funds to patient bills.
  • Responsible for coordinating and submitting all financial accounting procedures and financial audit requirements to organization’s financial department.
  • Responsible for updating grant letters annually to reflect changes to the Federal Poverty Level Guidelines and required documentation.
  • Responsible for updating Verification of Medical and Financial Eligibility Policy as needed.
  • Collects, prepares, and submits required quarterly programmatic data to the Department of Public Health in accordance to the Maternal and Infant Care Contract.
  • Responsible for coordinating collection and submission of required data for annual report to National Perinatal Information Center (NPIC).
  • Review quarterly NPIC reports with Maternal and Neonatal Outreach Coordinators and update appropriate parties, i.e., Women’s services leadership, OB/GYN physicians, QMS.
  • Attend weekly HROB Clinics and maintain monthly log of patients seen in HROB Clinic.
  • Develop and maintain a data base for annual information required under M & I Care contract with Georgia Department of Public Health.
  • Participates in the annual Georgia DPH annual audit
  • Serves as primary liaison and project lead on Quality Improvement initiatives as set forth by DPH and Georgia Perinatal Quality Collaborative (GaPQC).
  • Coordinate implementation of QI projects.
  • Responsible for coordinating GaPQC QI data collection and submission quarterly as requested by GaPQC.
  • Collaborate with Women’s Services leadership on implementation of QI initiatives.
  • Review and disseminate quarterly report information to appropriate parties.
  • Ongoing review of projects with Maternal Outreach Coordinator.
  • Assists with OB M&M committee case abstraction, review, and data collection.
  • Serves on assigned MESO committees related to Perinatal QI projects.
Certification Required
  • Current license as Registered Nurse by the Georgia Board of Nursing and certification in Inpatient Obstetric Nursing or related specialty.

  • Degree in Nursing
