  • Provide inpatient palliative care consultations and follow-up care for patients with serious illness across the AHN Palliative Care Program.

  • Manage initial and follow-up palliative care consultations, prioritizing patients by acuity and collaborating with a dynamic Interdisciplinary team .

  • Support complex symptom management, serious illness communication, goals-of-care discussions, hospice evaluations , and end-of-life care planning.

Scope of Practice / What You’ll Do

  • Provide initial and follow-up palliative care consultations for inpatient patients.

  • Partner with physicians for collaboration while functioning at the top of the APP scop e of practice.

  • Support patients and families through symptom management, hospice evaluations, goals-of-care conversations , and end-of-life planning.

  • Participate in interdisciplinary collaboration with physicians, APPs, hospice liaison support and palliative c ounselors.

  • Procedures are not required for this position.

Qualifications

  • Nurse Practitioner with appropriate Georgia licensure required or ability to obtain prior to start.

  • Acute Care NP, Primary Care NP certification required , as applicable to licensure and role scope.

  • New graduates will be considered.

  • 2 to 3 years of Hospice and/or Palliative Care experience, or related experience caring for patients with serious illness.

  • Hospice and Palliative Medicine (HPM) certification preferred.

  • Strong skills in symptom management, serious illness communication and end-of-life care .
