Scope of Practice / What You’ll Do
Provide initial and follow-up palliative care consultations for inpatient patients.
Partner with physicians for collaboration while functioning at the top of the APP scop e of practice.
Support patients and families through symptom management, hospice evaluations, goals-of-care conversations , and end-of-life planning.
Participate in interdisciplinary collaboration with physicians, APPs, hospice liaison support and palliative c ounselors.
Procedures are not required for this position.

Qualifications
Nurse Practitioner with appropriate Georgia licensure required or ability to obtain prior to start.
Acute Care NP, Primary Care NP certification required , as applicable to licensure and role scope.
New graduates will be considered.
2 to 3 years of Hospice and/or Palliative Care experience, or related experience caring for patients with serious illness.
Hospice and Palliative Medicine (HPM) certification preferred.
Strong skills in symptom management, serious illness communication and end-of-life care .
