Perform comprehensive assessments of high-risk members. Collaborate with primary care providers to develop and implement individualized care plans. Address, identify, and continuously reassess appropriate levels of care to ensure efficiency. Coordinate non-medical support such as housing and transportation to promote treatment plan adherence. Manage care transitions by coordinating with physicians, social workers, discharge planners, and claims professionals. Engage specialty resources as needed to resolve health issues effectively.
Maintain detailed records of clinical, functional, and outcomes data during management processes.
Monday-Friday shift with on-call responsibilities every three weeks, 8 am to 5 pm. On-site role located in Macon. Willing to accept a compact license; GA RN License required. No guarantee on hours; productivity requirement of 30 units. Mileage reimbursement at 0.43 cents per mile.
Education/Licenses NeededAssociates or Bachelor's Degree in Nursing or related field. Current, unrestricted state Registered Nurse license. Case Management certification.
