ProgramOverview:
The ICM Case Manager provides a comprehensive range of paraprofessional social services to patients, clients, and their families and will work across both the Bibb County and Baldwin County programs
. This role actively participates in treatment team planning, delivers crisis intervention when needed, and assists individuals in addressing personal and social challenges. Responsibilities include providing supportive counseling, serving as a liaison for social services, coordinating case management activities, and facilitating group skills training to promote the overall well-being of individuals in the program.
Monday-Friday 8:30am-5:00pm
Key Responsibilities:- Provide a broad range of paraprofessional social services to patients/clients and their families.
- Participate in treatment team planning and perform crisis intervention.
- Assist individuals in addressing personal and social challenges.
- Provide supportive counseling to consumers and families and/or serve as a liaison for social services.
- Perform case management duties, coordinating services and resources as needed.
- Facilitate group skills training to support client growth and development.
- Bachelor's degree in human services or a related field required.
- Minimum of two (2) years of case management experience.
- Two professional references from former employers (family members not accepted).
- Successful completion of background clearance and pre-employment drug screening.
- Case Management: Effectively manages and coordinates all aspects of a client's case, including assessment, planning, implementation, monitoring, and evaluation of services.
- Crisis Intervention: Responds promptly and effectively to individuals in acute distress, using techniques to stabilize and provide immediate support.
- Group Skills Training: Designs and facilitates group sessions addressing social and emotional needs, utilizing appropriate methods and resources.
- Empathy: Demonstrates genuine understanding and compassion for clients and families, fostering trust and rapport.
- Communication: Strong interpersonal skills to clearly convey information, actively listen, provide feedback, and collaborate with treatment teams, clients, and families in a culturally sensitive manner.
