IDD Program Case Manager

The IDD Program Case Manager is responsible for coordinating and monitoring services for individuals with Intellectual and Developmental Disabilities (IDD). This role ensures participants receive person-centered services that promote independence, community integration, health, safety, and achievement of individualized goals. The Case Manager serves as the primary liaison between individuals served, families, community resources, home managers and interdisciplinary team members.

• Coordinate and manage case services for individuals receiving IDD waiver and community-based services.
• Conduct comprehensive assessments and develop person-centered service plans based on individual needs, preferences, and goals.
• Monitor service delivery to ensure quality, compliance, and participant satisfaction.
• Maintain regular contact with individuals served family members, guardians, and service providers.
• Advocate for individuals' rights, choices, and access to appropriate resources and services.
• Assist individuals in accessing medical, behavioral health, vocational, educational, residential, and community support services.
• Complete required documentation accurately and timely in accordance with Georgia DBHDD, Medicaid, and organizational requirements.
• Monitor health and safety concerns and coordinate interventions as necessary.
• Participate in interdisciplinary team meetings and provide ongoing case updates.
• Ensure compliance with all state, federal, and agency regulations governing IDD services.
• Respond to crisis situations and provide support as needed.

• Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Special Education, Nursing, or a related field.
• Minimum of one year of experience working with individuals with intellectual and developmental disabilities, behavioral health, social services, or case management.
• Knowledge of person-centered planning principles.
• Strong organizational, communication, and documentation skills.
• Proficiency with electronic health records and Microsoft Office applications.
• Valid Georgia driver's license and reliable transportation.
• Ability to travel within assigned service areas.

• Experience with Georgia DBHDD programs and IDD waiver services.
• Experience supporting individuals with complex behavioral, medical, or social needs.
• Familiarity with Medicaid documentation and service authorization requirements.

• Understanding of Intellectual and Developmental Disability services and best practices.
• Ability to build positive relationships with individuals served, families, home managers and community partners.
• Strong advocacy and problem-solving skills.
• Ability to manage multiple priorities and deadlines.

• Ability to work independently and collaboratively within a multidisciplinary team.
