Responsibilities
  • Complete an initial comprehensive assessment of patient and family to determine hospice needs and provide a comprehensive physical assessment and history of current and previous illnesses.
  • Provide professional nursing care using all elements of the nursing process as defined in the State Nurse Practice Act.
  • Assess and evaluate patient status; write, initiate and regularly re‑evaluate the plan of care; revise as necessary.
  • Use assessment data to determine nursing diagnosis and develop a care plan that establishes goals and incorporates palliative nursing actions, involving patient and family in the planning process.
  • Initiate appropriate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed by the physician.
  • Provide counseling to patients and families to meet nursing and related needs.
  • Provide health‑care instructions, assist with activities of daily living, and facilitate patient self‑sufficiency and optional comfort care.
  • Act as case manager when assigned and coordinate care for the assigned caseload.
  • Maintain accurate and relevant clinical notes; record pain/symptom management changes and outcomes.
  • Communicate with physicians regarding patient needs, report changes, and obtain orders as required.
  • Coordinate care with community health providers and the interdisciplinary group.
  • Teach self‑care techniques and provide medication, diet and other instructions as ordered, while recognizing opportunities for health counseling.
  • Ensure a safe environment for patients and assist continuity of care.
  • Attend interdisciplinary group meetings.
  • Participate in on‑call duties as defined by policy.
  • Arrange for equipment and other necessary items and services.
  • Supervise ancillary personnel and delegate responsibilities.
  • Maintain personal growth and professional knowledge through continuing education and in‑service classes.
  • Fulfill accepted case assignments.
  • Participate in quality assessment and performance‑improvement teams.
Qualifications
  • Graduate of a National League of Nursing accredited nursing school.
  • Current California license and current CPR certification.
  • At least one year of professional nursing experience within the past three years, OR a baccalaureate nursing degree from an NLN accredited program and a current RN license; preferred two years of experience with at least one year in public health, home care, or hospice nursing.
  • Licensed driver with an insured automobile in good working order.
  • Responsible for supervising hospice aides; management experience not required.
  • Self‑directed, able to work with minimal supervision.
  • Excellent observation, verbal and written communication, problem‑solving, basic math, and nursing skills per competency checklist.
  • Capable of prolonged walking or standing; able to lift, position, transfer patients and lift supplies and equipment; capable of reaching, stooping, bending, kneeling, and/or crouching; adequate visual acuity and hearing for nursing skills.
Compensation and Scheduling
  • Hourly and paid‑per‑visit (PPV) hybrid model; salary option available.
