** Homeless Services Coordinator This position is responsible for providing homeless clients with direct case services including advocacy, support, goal setting, assistance in identifying options, evaluation of needs and information, and making necessary community linkages to support their self-sufficiency goals.

* EDUCATION and EXPERIENCE:

** Bachelor’s Degree in Social Work, Psychology, Counseling, or related field.
One year experience working in a similar or related field or equivalent of education/experience substitute.
** CASE MANAGEMENT DUTIES
** Manage ongoing case plans and update as clients meet goals or encounter new barriers.
Facilitate referrals to medical, mental health, substance use, legal, and employment services.
Provide direct support in navigating housing placements, lease agreements, and move-in coordination.
Assist with housing readiness and unit matching using PAD MISSION.
Offer personalized coaching on budgeting, independent living skills, and conflict resolution.
Prepare correspondence and documentation on behalf of clients as needed.
Act as an advocate at client appointments, hearings, and program meetings.
