Northgate Health & Rehabilitation is seeking an experienced and detail-oriented MDS Coordinator (RN or LPN) to join our clinical leadership team. If you have a strong understanding of the MDS process, Medicare and Medicaid reimbursement, care planning, and clinical documentation, this is an excellent opportunity to make a meaningful impact while working alongside a supportive interdisciplinary team.
As our MDS Coordinator, you will play a vital role in ensuring accurate resident assessments, maximizing reimbursement, maintaining regulatory compliance, and supporting exceptional resident outcomes.
Responsibilities- Coordinate and complete the Resident Assessment Instrument (RAI) process, including MDS assessments and Care Area Assessments (CAAs)
- Ensure timely and accurate completion and submission of all MDS assessments
- Develop and coordinate comprehensive interdisciplinary care plans
- Collaborate with nursing, therapy, dietary, social services, and physicians to ensure accurate clinical documentation
- Monitor Medicare and Medicaid documentation to support appropriate reimbursement
- Maintain compliance with CMS regulations and state guidelines
- Participate in Medicare meetings, quality improvement initiatives, and clinical audits
- Educate and support nursing staff regarding documentation requirements and regulatory standards
- Assist with survey preparation and regulatory compliance activities
- Current Alabama RN or LPN license in good standing
- Previous MDS Coordinator experience preferred
- Strong knowledge of RAI process, PDPM, Medicare, Medicaid, and CMS regulations
- Experience with skilled nursing or long-term care preferred
- Excellent assessment, documentation, organizational, and communication skills
- Ability to manage multiple priorities while meeting regulatory deadlines
- Experience with electronic medical records is preferred
