The Specialty Care Clinic is seeking a compassionate and dedicated Case Manager to join our interdisciplinary team serving individuals living with HIV.

This position plays an important role in helping clients overcome barriers to care and achieve positive health outcomes. The Case Manager will provide culturally responsive, trauma-informed services, including psychosocial assessments, individualized service planning, crisis intervention, advocacy, and connections to community resources.

Working alongside medical and other supportive services staff, the Case Manager will help coordinate whole-person care with the goal of supporting clients in achieving and maintaining viral suppression and improving their overall quality of life.

Examples of duties
  • Conduct comprehensive psychosocial assessments and identify client needs, strengths, and barriers to care.
  • Verify and maintain client eligibility for Ryan White services.
  • Develop and update individualized service plans using a strengths-based approach and in accordance with Ryan White Case Management Standard Operating Procedures.
  • Advocate for clients and assist in addressing barriers that may interfere with access to healthcare and supportive services.
  • Connect clients with appropriate community resources and support services.
  • Collaborate with medical and interdisciplinary team members to develop interventions that support treatment adherence and improved health outcomes.
  • Provide crisis intervention and coordinate referrals for appropriate follow-up care.
  • Assist clients with applications for medication assistance programs.
  • Maintain knowledge of current evidence-based practices and incorporate appropriate interventions into client care.
  • Maintain timely, accurate, and thorough documentation of assessments, interventions, communications, and services provided.
  • Adhere to the NASW Code of Ethics, clinic protocols, and applicable program requirements.
Qualifications

Master's degree in social work and counseling from an accredited college or university and eligibility to be licensed.
