Nursing Assistant

We are seeking a compassionate and caring Nursing Assistant to join our team at SYNERGY Home Care of East Haven in Madison, Connecticut. • Assist clients with daily living activities, such as bathing, dressing, and grooming, in a safe and respectful manner
• Provide personal care, including toileting, transferring, and ambulation assistance as needed
• Monitor and report changes in clients' condition to licensed nursing staff
• Assist with medication reminder.
• Develop and maintain positive relationships with clients and their families, fostering a supportive and inclusive environment
• Collaborate with interdisciplinary teams to develop and implement individualized care plans, promoting effective communication and coordination of care
