infants in BLOOM is a mission-driven, woman-owned,and woman-led pediatric in-place care organization serving vulnerable infants and caregivers in rural southeast Indiana. We provide comprehensive, in-home pediatric care from birth through 36 months, meeting families where they are - in their homes, in their communities, and at their most critical developmental window.

And if an infant’s placement changes, our care moves with them to ensure absolute clinical continuity.

BLOOM serves as the primary pediatric provider forourfamilies, including administration ofall required vaccinationsas part ofa state pilot. We work in close partnership with local health departments and the Department of Child Services (DCS).

We are seeking aNurse Practitioner to join our growing clinical teamwithconsiderable experience in newborn and early childhood care.

You will serve patients across a seven-county region in southeast Indiana, conducting in-home visits and telehealth appointments on a schedule that works for you.

• Provide primary pediatric care for infants and young children from birth through 36 monthsthroughin-home clinical visits,whilefollowinga novelcare plan. Telehealth visits as needed.
• Assessment and support of caregiver wellbeing through coaching and education.
• Collaborate with BLOOMteam,DCS,CASA,local healthdepartmentsand other community resources.
• Document clinical encounters using our modern, AI-assisted technology platform.
• Participate in care coordination,trainings, team meetings,and call rotation.

• Licensure:
  Active, unrestricted Indiana NP or PA licensure (or immediate eligibility).PNPpreferred, FNP or NNP.
• Certifications:
  
  Board Certified or Board Eligible in one of the following specialties:
• Pediatric Nurse Practitioner (PNP) — Highly Preferred
• Family Nurse Practitioner (FNP) with strong early childhood experience
• Neonatal Nurse Practitioner (NNP) with early childhood experience
• Logistics:
  
  Valid driver’s license, reliable transportation, and willingness to travel within the 7-county southeast Indiana region.
• Experience or genuine interest invulnerablenewbornsthrough
  36 months.
• Comfort with homevisits, rural health,and community-based care.
• Strong communication skills and alignment with BLOOM's mission-driven culture.

• Minimum commitment of just 10 hours per week.

• No daily commute requirement — visits are strategically planned across the region.
