Licensed Practical Nurse (LPN) – Rehabilitation

Job Type: Full Time | Nights

Licensed Practical Nurse joining our team plays a vital role in delivering compassionate, patient‑centered care in a 40‑bed inpatient rehabilitation setting.

Responsibilities include:

• Demonstrates knowledge of medications and correct administration based on patient age and clinical condition.
• Reduces the potential for medication errors by following correct medication procedures.
• Performs patient care considering needs specific to the standard of care for patients’ age; includes geriatric and general inpatient rehabilitation patients.
• Under R.N. supervision, assists with the assessment of all patients and reassessments as per policy, identifies and initiates appropriate rehabilitation nursing interventions.
• Under R.N. supervision, performs timely and accurate QI assessments.
• Carries out the plan of care as indicated by the patient’s needs and response to treatment; evaluates overall plan daily for effectiveness; updates interdisciplinary care plan accordingly.
• Interacts professionally with patient/family and involves patient/family in the formation of care plans.

• Graduate of an accredited licensed practical or vocational nurse program.
• Current state licensure as Licensed Practical/Vocational Nurse.
• CPR/BCLS certification; ACLS preferred.
• IV Certified required or obtainable within 6 months.
• Minimum six months medical/surgical experience in an acute care setting preferred.
• Excellent oral and written communication and interpersonal skills.
