40.00 - Salary Range: $38.50 - $55.00
- Sign On Bonus: $15,000
The Clinical Care Leader (CCL) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care in the home setting. The CCL collaborates with inter‑professional colleagues to plan, implement and evaluate care, documents resident history, symptoms, medications and care provided in compliance with applicable standards and regulations.
Responsibilities- Use technology to enhance quality resident care and collaborate with inter‑professional team members.
- Delegate to and supervise unlicensed personnel, assume overall nursing accountability for a resident assignment and serve in leadership roles on department/organizational task forces and committees.
- Demonstrate competency and practice within the full scope of nursing expertise and utilize appropriate age and population‑specific standards as designated in the specific care area or unit.
- Communicate effectively to collaborate, negotiate and resolve conflicts, situations, and/or issues with peers, inter‑professional colleagues, residents and families.
- Apply strong clinical reasoning and critical thinking skills to analyze data and respond appropriately to changing situations.
- Coordinate care, conduct resident assessment, provide resident education, triage, and perform various other nursing interventions.
- Operate within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules for the state of practice and licensure.
- Bachelor's degree in Nursing strongly preferred.
- Graduate from an accredited nursing program, including but not limited to those accredited by the Commission on Collegiate Nursing Education, Accreditation Commission for Education in Nursing, or National League for Nursing.
- Minimum of two years’ clinical experience preferred.
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possesses multistate licensure if in a Nurse Licensure Compact state.
- Obtains and subsequently maintains required department specific competencies and certifications.
- Basic Life Support (BLS) required.
- Resident Assessment Instrument (RAI) process experience preferred, but not required.
