Position: PT / Licensed Physical Therapist

Responsibilities
  • Perform thorough evaluations of patients in inpatient and outpatient settings, crafting tailored treatment plans to meet individual needs.
  • Apply physical therapy methodologies aimed at enhancing mobility, strength, balance, and overall functionality.
  • Work in unison with nursing staff, physicians, case managers, and other rehabilitation professionals to achieve the best outcomes for patients.
  • Foster educational relationships with patients and their families, setting clear therapeutic goals and guiding them through discharge planning and at-home exercises.
  • Maintain accurate documentation of evaluations, treatments, and patient progress in line with facility protocols and regulatory requirements.
  • Engage in multidisciplinary care rounds and interdisciplinary meetings to facilitate seamless care continuity.
Qualifications
  • Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution.
  • Current state license to practice as a Physical Therapist is essential.
  • Preferred CPR certification.
  • Specialized credentials in fields such as neurology or geriatrics are a plus.
  • Prior experience is preferred, showcasing ability to manage complex medical and mobility challenges.
  • Excellent communication and collaboration skills, creating a positive care environment.
  • Comfort and proficiency with digital charting and clinical documentation systems.
