Relient Health is seeking licensed Physical Therapists (PTs) for full‑time, in‑person opportunities within established rehabilitation departments across Wisconsin. These are full‑time, direct‑hire, permanent placement PT positions (not travel, not contract, not PRN).
Our openings include positions in Skilled Nursing Facilities (SNFs), Assisted Living Facilities (ALFs), retirement communities, hospitals, outpatient clinics, pediatric settings, and home health, depending on the specific location.

- Licensed Wisconsin Physical Therapists
- PTs relocating to Wisconsin
- New graduates from accredited DPT programs pursuing Wisconsin licensure
- PTs seeking experience in SNF, hospital/acute care, IRF, outpatient, home health, or mixed‑setting rehab roles
This role is specific to licensed Physical Therapists working within a rehabilitation department.
This is not a CNA, nursing assistant, medical assistant, or therapy aide position.
💰 Compensation & BenefitsCompetitive salary range: $40-52/hr (Compensation varies by market, setting, and experience)

- Evaluate patients and develop individualized treatment plans
- Provide skilled physical therapy services in a skilled nursing, hospital/acute, inpatient rehab (IRF), outpatient, or home health environment
- Collaborate with interdisciplinary rehab and nursing/medical teams
- Supervise and partner with Physical Therapist Assistants (PTAs)
- Document care in EMR systems per state, federal, and facility standards
- Promote functional independence, safety, and quality of life
- Graduate of an accredited Doctor of Physical Therapy (DPT) program
- Active Wisconsin PT license, or eligibility/pending licensure
- Ability to work full‑time in one of the listed Wisconsin markets
- Authorized to work in the United States
