Just Dane is seeking two part-time (20 hours per week each) Community Health Workers (CHW) to serve as trusted guides for learning, growth and wellness for current and former Healing House guests. CHWs will foster community connections and link families to vital health and social resources both pre and post-discharge from Healing House. Using resident-identified priorities to guide their work, CHWs provide wellness coaching, health education, system navigation, wellness outcome tracking and advocacy while supporting guests in building self-efficacy and achieving their personal goals.

CHWs are Healing House staff members who work both in Healing House during the guests’ stay, and post-discharge from Healing House in the community, collaborating closely with former Healing House guests, staff, and community-based partners. The role blends direct support, community health education projects, documentation, and participation in program evaluation and professional development.

Responsibilities
  • Conduct and document initial assessments and collaborative health/wellness goal setting with guests.
  • Provide holistic, trauma-informed wellness coaching, including mental health support, motivation, accountability, and encouragement when appropriate.
  • Maintain a caseload of approximately 8 guests, spending an average of 1-2 hours per week per guest.
  • Revisit and update guest goals regularly, tracking progress and outcomes.
  • Assist guests in navigating health care, social services, and community resources while building self-advocacy skills and independence. These functions occur both pre and post-discharge from Healing House.
  • Advocate for residents when needed, while nurturing long-term self-efficacy.
  • Provide culturally appropriate health information and education in collaboration with clinical staff and community partners.
  • Conduct ongoing visits with Healing House guests post-discharge (home, shelter or other community setting) to provide continued assistance with healthcare access, health advocacy, referral and linkage to other services, assist in tracking health access and health outcomes post-discharge and supports.
  • Participate in and help facilitate community health promotion projects/groups, which may include:
    • Vaccination clinics
    • Wellness Groups
    • Healthy housing advocacy
    • Disease-specific education (e.g., diabetes)
    • Planning or co-facilitating support groups (e.g., parent cafes, grandparents, emotional wellness)
    Document guest goals, interactions, wellness journeys, and outcomes in a timely and accurate manner.
  • Participate in program evaluation and data tracking activities to support reporting and continuous improvement.
  • Attend regularly scheduled CHW team meetings and supervision check-ins for caseload coordination, guidance, and problem-solving.
  • Maintain regular office hours and attend staff meetings, Healing House Advisory Committee Meetings, and other organizational meetings as requested.
  • Work collaboratively with staff, volunteers, and community-based organizations to strengthen guest support systems within the community.
  • Maintain strict confidentiality and follow all organizational policies and procedures.
  • Stay current on available community, county, state, and federal resources through ongoing self-education and professional development.
  • Participate in CHW network meetings and training opportunities.
Minimum Qualifications
  • Certified Community Health Worker
  • Strong Interpersonal communication and relationship building skills
  • Basic computer proficiency
  • Experience working in a diverse, multicultural, community based setting
  • Supervision

The Community Health Worker reports to the Healing House Manager

Schedule

Generally Monday through Friday between the hours of 8 a.m. – 5 p.m. with some flexibility to meet the needs of families post-discharge from Healing House.
