Organ and Tissue Donation Aftercare Coordinator

Responsibilities

• Provide bereavement and grief support directly to donor families
• Support, guide, and provide direction for the donor family aftercare program of UW Organ and Tissue Donation (UW OTD)
• Serve as a key liaison between donor families and various stakeholders, including hospital staff, partner organizations, and community groups
• Gather donor family feedback, provide observation and feedback, and collaborate with leadership to engage in process improvement process
• Maintain and facilitate the private donor family services social media group page

• Bachelor's Degree in social work, nursing, grief/bereavement care or related field (Required)
• Master's Degree in social work, nursing, grief/bereavement care or related field (Preferred)
• 2 years of experience in a healthcare setting (Required)
• Experience in organ procurement or transplantation (Preferred)
• Experience in end of life settings, Hospice, or ICU setting (Preferred)
• Must possess a current, valid driver’s license issued by the individual’s state of residence. Must be at least 21 years old and have a minimum of three (3) years of driving experience to operate a Personal Vehicle, Patient/Visitor Vehicle, or standard UW Health Fleet Vehicle. (Upon Hire Required)

Demonstrates expertise in providing bereavement and grief support to donor families, with a strong focus on collaboration with healthcare stakeholders and process improvement. Holds relevant educational qualifications and experience in healthcare settings, particularly in end-of-life care.
