• Duration: 13 weeks
• Shift Duration: 12 hours
• Shift Time: Day shift, 6:00 AM - 6:30 PM

• Charting System: Epic preferred
• Orientation: Multiple orientation dates available prior to start

• 2+ years of experience in skilled nursing, inpatient rehab, med-surg, or another 24/7 skilled care setting
• Current BLS certification
• CNA or Patient Care Technician (PCT) background strongly preferred
• Experience with electronic medical records;
  Epic experience preferred
• Comfort working in a high-acuity rehab environment with a team-first mindset
• Strong attendance, reliability, and flexibility are essential
• Ability to work with adult patients receiving inpatient rehabilitation services
• Willingness to learn new skills and assist across the unit as needed

• Provide direct patient care and clinical support in an inpatient rehabilitation hospital setting
• Assist patients with mobility, hygiene, transfers, toileting, and activities of daily living
• Respond promptly to call lights and support patient safety throughout the shift
• Monitor and assist with common rehab needs such as mobility equipment, vital signs, blood glucose checks, and charting support
• Support care for patients recovering from stroke, spinal cord injury, amputation, traumatic brain injury, orthopedic injury, and other complex conditions
• Help with feeding tube care, swallowing precautions, oxygen support, Foley care, wound care, and infection precautions as directed
• Collaborate closely with nurses and the interdisciplinary care team to promote patient recovery and rehabilitation goals
• Maintain a professional, team-oriented approach in a fast-paced hospital environment
• Follow facility policies related to attendance, breaks, scheduling, and dress code
