Job Summary:
Are you a compassionate and dedicated Mental Health Provider looking to make a direct impact on the lives of older adults? Care Companion/Social Worker at Carease Health - WI is seeking a passionate and experienced Care Companion/Social Worker to join our Collaborative Care team. In this role, you'll provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.
As a key member of our team, you'll work with psychiatric professionals to ensure comprehensive care, promoting the well-being and quality of life for our patients.
Visit designated facilities in accordance with the schedule set with your supervisor
Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
Document visit notes and maintain detailed records of interactions and interventions after each visit
Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
Strong communication, documentation, and relationship-building skills

Part-time opportunities with approximately 10 hours per week during Monday-Friday, within regular 8-5 hours
Flat salary: $17,000 - $19,000 ANNUALLY
