Catholic Charities has an immediate opening for a full time Medical Case Manager. The Medical Case Manager is responsible for coordinating and managing appropriate services as they relate to clients’ health and medical needs; and assists with supervision and support of residents in 5 Door Recovery’s residential programs. The Medical Case Manager is also responsible for prompt, clear, and thorough communication the Manager of 5 Door Recovery and clinical staff.

Services delivered by the Medical Case Manager are consistent with State and Federal regulations, the established care delivery model within the individual’s scope of practice, and the general operating philosophy and agency values.

Starting pay is $22.74 per hour, commensurate with experience.

Principal Accountabilities Management
  • Provides a welcoming, positive, and supportive presence for all clients, demonstrating interest in and availability to clients.
  • Assesses the health care needs of newly admitted residents, including reviewing their medical history, treatment of dental and or other neglected health problems; administering pregnancy tests if requested; obtaining and reviewing discharge orders for clients transferring from another facilities; and obtaining release of information forms to communicate with providers as needed.
  • Obtains and reviews lists of prescribed medications along with signed medication orders from client's physician(s); checks the medication list for prohibited medications; compares the prescribed medications with medications brought by the client; counts the pills and enters the date on the Medication Administration Records; coordinates with pharmacies to obtain and refill medications for clients as needed.
  • Collaborate with primary counselors to assist with case management and documentation of processes, especially as they relate to medical, health, and wellness issues.
  • Performs comprehensive case management services within a substance abuse treatment program for adults which also functions as a community-based residential facility (CBRF), including, if needed, assisting clients in applying for medical insurance, assisting clients in obtaining medical and dental care, and arranging for transportation to medical and dental appointments.
  • Administers urinalysis and breathalyzer tests as needed and documents results. Review these test logs on a weekly basis to assess accuracy of documentation.
  • Reviews the Medication Administration Records weekly to determine accuracy of the documentation and the need for refills.
  • Takes the lead role for oversight of medication-related issues, including education and counseling with clients regarding side effects and monitoring for adverse effects of prescribed medications.
  • Documents interactions with clients including observations related to clients' attitudes related to treatment and recovery, interactions with peers, compliance with rules and regulations, and other relevant information.
  • Assists in monitoring agency compliance with applicable policies, procedures, and guidelines.
  • Performs other tasks as assigned.
Program Management
  • Reviews incoming client's insurance for coverage of applicable services.
  • Orders medical supplies, including tuberculosis testing supplies, drug testing supplies and over the counter medications for client use.
  • Collects co-payments from clients for pharmacy orders.
