In-Home WV Traveler, Nurse Practitioner or PA

Job Summary

Signify Health, part of CVS Health®, is hiring a Full‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations and related services to patients across a defined coverage area. The role requires extensive travel, multi‑state licensure, and a commitment to building trusted relationships with patients in their homes.

• Conduct in‑home visits lasting approximately 45 minutes, including medication and medical history review and a physical evaluation.
• Order and perform optional diagnostic tests (e.g., diabetic eye exams, spirometry, bone density screening) as directed by the patient’s health plan.
• Use an iPad and an intuitive clinical workflow to complete each evaluation.
• Travel up to 100% of the time, including overnight stays, car rentals, and flights as required by the coverage area.
• Maintain active, unrestricted licenses in all required states.
• Provide virtual visits as needed.
• Represent Signify Health professionally and contribute to a national network of purpose‑driven clinicians.

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA).
• Active, unrestricted license(s) in all coverage states.
• Board certification (required).
• Ability to obtain and maintain multi‑state licenses in the coverage area.
• Flexibility to cover all counties within the state and other business‑required areas.
• West Virginia coverage includes Beckley, Charleston, Cumberland, Elkins, Huntington, Martinsburg, Morgantown, Parkersburg, and Wheeling.

• Experience with in‑home or concierge‑style health evaluations.
• Experience with diagnostic and preventive services such as spirometry, DEE, and bone density screening.

• Typical pay range: $78,331.00 – $ (base hourly rate or base annual salary).
