Position: Nurse Care Coordinator (RN/LPN) - Mon-Fri, Flexible Schedule

Full Time Nurse - RN or LPN (Full‑time, 32+ hours per week).
Current Openings: Nurse (Region from Wisconsin Dells, WI to Madison, WI).
Shift: Other, see hours.
Hours: 8 am–4 pm, Monday–Friday.
Salary: $35 hr + (based on experience).

Senior Span Management is seeking RN or LPN Nurses to join our team in several communities including Wisconsin Dells, Reedsburg, Portage, Baraboo, Sauk City, Lodi, Cross Plains, Middleton, Verona and Mt. Horeb. We are ever expanding and proactively engaging with prospective staff!

Our Nurse Care Coordinators play a vital role in enriching the lives of our residents by coordinating individualized care plans, guiding care teams, and ensuring each of our seniors receives the highest level of clinical and compassionate support. They are accountable for participating in assessment, planning, implementation and evaluation of resident care, as well as collaborating with the RN Administrators, care staff, physicians, and family members to ensure each resident maintains or attains the highest physical, mental and psychosocial well‑being.

Responsibilities
  • Assess, plan, evaluate and direct person‑centered care for frail seniors.
  • Oversee medication administration, training, injections and blood draws.
  • Create and update each resident’s Plan of Care, communicating changes in condition to physician, family members and team members in a timely manner.
  • Ensure that each resident’s physical, mental and spiritual needs are met by working with and monitoring care staff and serving as assistant Administrator when delegated by the Administrator in their absence.
  • Document in the electronic health record in a complete and timely manner.
Qualifications
  • Registered Nurse or LPN license.
  • 2+ years of experience.
  • Excellent nursing skills and compliance with the State of Wisconsin regulations.
  • Strong communication, interpersonal and leadership skills.
