Registered Nurse Alt Shift (RN Alt Shift)
Location:
Madison, WI 53711
Job Type: Full-Time/Part-Time (0.8 FTE, 32 hours/week)
Shift: NOC (weekdays, 7p‑m to 4a‑m) with on‑call rotation, weekend and holiday shifts as needed
Travel:
Approximately 50 miles from home
Salary:
Competitive hourly rate with mileage reimbursement
Sign‑On Bonus: $10,000
Key Responsibilities- Patient and Family Assessments
- Assess patients and families on physical, pain, psychosocial, emotional, spiritual, environmental, and educational needs as outlined by the plan of care.
- Collect data from the patient, family, significant others, and interdisciplinary team members.
- Identify, interpret, and document assessment data from nursing interviews, physical assessment, and other health data.
- Document and report significant changes or unusual incidents to the appropriate IDT member.
- Collaborate with IDT and primary care provider to create HES for determination of hospice eligibility.
- Patient Visits
- Respond to patient and family needs or changes in condition requiring acute intervention.
- Provide interventions, treatments, and pain management to meet patient and family needs.
- Evaluate effectiveness of pain management or other interventions and adjust as necessary.
- Educate patients and families on pain, symptom management, end‑of‑life care, etc., and adjust education based on effectiveness.
- Plan of Care
- Formulate, implement, and revise individualized POC in collaboration with IDT, patient, and family.
- Develop measurable outcome goals and establish priorities in planning and providing care.
- Read, understand, and follow the plan of care; document interventions, outcomes, and condition changes in the EHR.
- Delegate visits to appropriate disciplines and manage visit sets.
- Oversee work of Certified Nursing Assistants (CNA) and Licensed Practical Nurses (LPN).
- Miscellaneous
- Participate in meetings to plan and evaluate patient/family services.
- Precept new staff and students.
- Current, unrestricted Registered Nursing License in the state of employment.
- One year of nursing experience preferred.
- Valid driver’s license, auto insurance, and reliable vehicle.
- Must be at least 18 years of age.
- Basic Microsoft Office skills.
- Intermediate electronic health records skills.
- Ability to use proper body mechanics, safe patient handling, and standard precautions.
