As a Home Health Clinician, you’ll provide skilled nursing care to patients in Assisted Living Facilities (ALFs) and in their homes—helping them live healthier, more independent lives.

• Holds (or can obtain) a current Wisconsin RN license

• Serve as Case Manager, coordinating patient care plans
• Conduct thorough assessments and develop individualized treatment plans
• Collaborate with patients, families, and clinical staff to maximize outcomes
• Document care clearly, including discharge summaries
• Educate patients and caregivers on safe discharge planning

• Pay starts at $38.00/hr but can increase on experience
