Work Schedule:

100% FTE, Monday – Friday, 8:00 AM – 5:00 PM. No holidays or call.

• Work collaboratively within a multidisciplinary health team.
• Conduct routine physical exams.
• Conduct focused health histories and physical exams of patients with acute and/or chronic health problems.
• Order and interpret appropriate diagnostic testing.
• Establish a working diagnosis and develop a treatment plan.
• Prescribe medications and/or treatments and monitor patient’s response.
• Prescribe and treat within the full scope of primary care.

• If Nurse Practitioner:
  Master’s or Doctorate’s degree from an accredited Nursing Program.
• If Physician Assistant:
  Bachelor’s or Master’s degree from an accredited PA Program.

• Minimum – State of Wisconsin NP or PA licensure.
• For Nurse Practitioner: active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice; active board certification as an NP in adult/gerontology or family practice.
• For Physician Assistant: active PA license, including prescriptive authority, in state(s) of practice; active PA board certification.
• All Nurse Practitioner and Physician Assistant applicants must have an active DEA license or the ability to apply for one prior to or upon hire.
