RN Thoracic Oncology Navigator

• Provide comprehensive cancer care for patients and caregivers throughout all phases of cancer care
• Assess barriers to care, psychosocial needs, and learning needs
• Advocate for and support patients and caregivers collaboratively with teams and support services
• Facilitate timely and coordinated transitions of care
• Provide anticipatory guidance and reinforce patient and caregiver education
• Guide patients and caregivers through shared decision-making
• Provide mental and emotional support throughout the continuum of cancer care
• Participate in multidisciplinary tumor board meetings
• Participate in continuous quality and process improvement initiatives

• Bachelor's Degree in Nursing (BSN) required
• 3 years of relevant clinical nursing experience required
• Registered Nurse license in the state of Wisconsin required upon hire
• Basic Life Support/CPR required upon hire
• Master's Degree in Nursing, Social Work, Public Health, Healthcare Administration, or related field preferred
• 1 year of relevant experience in the specific patient population being supported preferred
• Applicable oncology certification based on patient population preferred

Demonstrates expertise in providing comprehensive cancer care, including patient advocacy, psychosocial support, and education. Skilled in facilitating transitions of care and participating in multidisciplinary teams to enhance patient outcomes.
