Job Summary

In this full-time role, you will be a key member of St. Vincent de Paul – Madison’s Social Services team, supporting programs that promote long-term stability and well-being for individuals and families in our community. Core responsibilities include assessing and supporting program participants, developing and monitoring individualized case plans, coordinating services and referrals, maintaining accurate and confidential records, and collaborating with internal teams and community partners.

This position plays an essential role in strengthening families, advancing program outcomes, and furthering the mission of St. Vincent de Paul through compassionate, client-centered service.

What You’ll Do
  • Assess potential program participants and orient those selected into the program.
  • Prepare and monitor case plans for up to 20 newly housed families to set, work toward, and attain goals that promote each family’s well-being.
  • Assist in facilitating connections between families and other available SVdP - Madison services, and provide appropriate referrals to external agencies when needed. Additionally, support program participants in utilizing these services effectively and facilitate their connection with external agencies as necessary.
  • Work with each client to manage their budget and coordinate payment of the expenses deemed appropriate with the Senior Director of Client Services.
  • Maintain accurate, confidential case records in compliance with organizational policies and applicable privacy standards.
  • Offer/coordinate adult education programming on topics such as financial literacy, parenting skills, pursuing further education, securing jobs, and/or other life skills to meet client goals.
  • Collaborate with Social Services Director and Senior Director of Client Services to develop, maintain, and report program metrics related to family wellbeing, family finances, school participation/success, mental health, and/or other important key indicators. Utilize the gathered data to enhance both program effectiveness and systemic processes.
  • Provide additional St. Elizabeth Ann Seton Program support to Social Services Director
Microlending Program
  • Assess client applications, conduct preliminary interviews for eligibility, facilitate loan process within SVdP and lending partner.
  • Consistently communicate to applicants regarding their eligibility, providing clear and concise explanations
  • Conduct monthly follow-up with loan clients and train and support volunteers engaged in loan monitoring activities
  • Provide additional Microlending Program support to Social Services Director
  • Share program impact stories with SVdP - Madison leaders to support and help grow community awareness, while maintaining participant confidentiality.
  • Represent the programs at Homeless Services Consortium or other community collaborations and advocate for policies that support the needs of participants in these programs as such advocacy is approved by the Board of Directors.
  • Participate in an atmosphere of teamwork and collaboration, listening, fostering dialogue, and facilitating joint problem solving while supporting an inclusive and equitable organization.
  • Adhere to the Safeguarding Policy by ensuring timely completion of mandatory training, staying updated on relevant training modules, and undergoing the necessary background checks to maintain compliance.
What You Can Bring Required
