Clinical Case Manager

Community Care Resources, Inc. (CCR) has been providing Out-of-Home Care for Treatment Level youth for over 35 years throughout the entire state of Wisconsin.

We are currently recruiting for Clinical Social Workers to grow our existing team. Join an experienced agency that is committed to both the youth and families it serves as well as its staff. Social Workers at CCR enjoy the benefit of supervisors who have worked in the field and have over 20 years of experience. Social workers are provided with autonomy, support and are allowed flexibility in performing their job requirements.

We are looking for candidates for a part-time/contract positions with the potential for growth to full-time.

Specific areas of coverage are needed for homes in Dane County, Dodge County, and Jefferson County.

• Provide case management services, direct services and facilitating services for youth placed in CCR foster homes.
• Face-to-face contact with foster parents a minimum of once per week
• Face-to-face contact with foster children a minimum of once per week
• Facilitate treatment planning meetings for youth in care
• Complete all required documentation, assessments and reports as directed by CCR and required by DCF
• Complete annual foster parent evaluations and provide educational support as needed for assigned foster families
• Attend monthly staff meeting at main office in Middleton
• Provide after-hours On-Call coverage as assigned
• Collaborate/Team with partner agencies

• Master’s Degree in Social Work or Human Services related field (In-process applicants will also be considered).

  Required:
  
  LCSW, LPC or APSW or equivalent. Must be a licensed social worker in the State of Wisconsin.
• Minimum of 2 years’ experience in foster care and social work case management.
• Experience with children and adolescents in a mental health, residential treatment, or other relevant experience will be considered for the right candidate.
