About The Community Resource Center

Neighborhood House’s Community Resource Centers support individuals and families as they address immediate needs and pursue greater social and economic well-being. The Centers provide welcoming, culturally responsive spaces where participants can access crisis support, connect with community resources, and receive individualized assistance with both short-term needs and longer-term goals.

Community Resource Navigators partner with participants to identify priorities, develop strategies, coordinate referrals, and support progress toward greater stability. The Community Resource Navigation team works closely with programs across Neighborhood House and with external partners to help participants access a broad range of services and opportunities.

Position Summary

The Community Resource Navigator partners with individuals and families to identify strengths, needs, barriers, and goals; develop individualized strategies; and connect participants with resources that support immediate needs and longer-term stability. The position provides culturally responsive coaching, case management, crisis support, safety planning, referrals, and coordination with Neighborhood House programs and community partners.

The Community Resource Navigator also supports the daily operation of the Community Resource Center, assists participants in accessing center resources, coordinates family activities, maintains accurate participant records, and contributes to program evaluation and reporting.

Essential Duties And Responsibilities Coaching & Case Management
  • Conduct intake and ongoing meetings with participants.
  • Conduct comprehensive participant assessments to identify strengths, needs, barriers, risks, and goals.
  • Identify and evaluate community resources that align with participant needs and goals.
  • Partner with families to develop and implement individualized strategies for accessing basic resources, strengthening social supports, and advancing family goals.
  • Support participants in developing safety plans and provide culturally responsive services consistent with those plans and established program protocols.
  • Coach families at risk of out-of-home placement or displacement to strengthen parent-child relationships, parenting skills, and family stability.
  • Coordinate language access and adapt communication approaches to ensure meaningful participation by individuals whose preferred language is not English.
Community Resource Center Operations
  • Receive, organize, stock center donations
  • Recycle and discard unwanted items
  • Assist with assigning volunteer and intern duties
  • Assist participants to shop in center pantries
  • Complete referrals to partnering resources such as My Very Own Bed, Bridging, MN Sure, or College Bound
  • Coordinate monthly family events for families in collaboration with schools and other organization departments
Data and Evaluation
  • Collect and enter all required data using Client Track software.
  • Implement program evaluation tools and track progress using relevant database(s).
  • Maintain confidentiality of program participants in accordance with data privacy laws, standards, and policy.
  • Prepare or assist with preparing program reports.
Experience, Knowledge, And Skills Required Experience and Skills
  • Bachelor’s degree in social work, human services, or related field, or four years of related work experience in family case management and/or program management.
  • Working knowledge and experience working with individuals and families, including working knowledge of immigrant/refugee needs and familiarity with community resources.
