Lutheran Social Services of WI & UP MI seeks a Service Facilitator to coordinate community-based services for individuals and families. In this role, you will assess client needs, develop person-centered service plans, and connect clients to behavioral health, housing, and supportive resources. You'll collaborate with multidisciplinary teams, monitor progress, maintain accurate documentation, and ensure trauma informed, culturally responsive care. This position offers training, supportive supervision, and opportunities to innovate in service delivery while advancing LSS's faith inspired, inclusive mission.
Responsibilities
- Conduct comprehensive assessments of client strengths, needs, and goals.
- Develop and implement person-centered service plans in collaboration with clients and families.
- Coordinate and monitor community-based services, including behavioral health, housing, and supportive resources.
- Maintain accurate, timely documentation and electronic records in compliance with agency and regulatory standards.
- Collaborate with internal and external providers to ensure integrated, trauma-informed care.
- Advocate for clients, providing referrals and support to navigate community systems.
- Monitor client progress, adjust service plans, and conduct regular follow-ups.
- Participate in team meetings, supervision, and ongoing training.
- Respond to client crises using de-escalation and crisis intervention strategies.
- Uphold LSS values of inclusivity, dignity, and respect in all client interactions.
