The Case Management (CM) Supervisor oversees supportive services provided to program participants living in scattered site Porchlight Housing. The CM Supervisor provides high-quality individual supervision to Case Managers, including resources, training, feedback, and support. The CM Supervisor also maintains a participant caseload, delivering direct case management services.

  • Bachelor's degree in social work or closely-related field, or equivalent knowledge and experience.
  • Experience in a nonprofit social service agency required. Supervisory experience desired.
  • Strong crisis management skills.
  • Willingness to work alongside a diverse population with varying strengths and challenges.
  • Excellent written and verbal communication. High level of organization.
  • Ability to build consensus, motivate and obtain willing cooperation from others.
  • Proficiency in Microsoft Word, Excel, Outlook, and Teams. Ability to adapt to new technology.
  • Driver's license, insured personal vehicle.
  • ESSENTIAL RESPONSIBILITIES/DUTIES Staff & Program Supervision:
  • Collaborates with the Director of Supportive Services to manage case management operations. Ensures consistent communication and processes regarding coordinated entry, program enrollment, case management procedures, and staff support.
  • Conducts regular supervisory meetings with assigned staff to identify and pursue their employment objectives. Provides consistent and equitable mentorship, training, support, and guidance. Offers constructive or corrective feedback on performance as necessary.
  • Regularly reviews participant files and case notes, evaluates data quality, and formulates improvement plans as necessary.
  • Reports staff performance issues, incidents of misconduct, or other concerns as needed to Human Resources and the Director of Supportive Services to determine next steps.
  • Promptly approves biweekly payroll and monthly mileage reimbursement. Approves all time-off requests and ensures staff notify participants of scheduled days off.
  • Assists Human Resources in facilitating the recruitment process for new and open positions, including reviewing applications, and interviewing candidates.
  • Onboards and orients new staff to the agency, program, and their specific job duties. Coordinates and delivers ongoing training to case management staff on best practices, including housing first, trauma-informed care, and harm reduction.
  • Facilitates monthly case management team meetings. Guides collaborative problem-solving and schedules guest speakers. Coordinates biannual staff appreciation luncheons.
  • Works collaboratively with community partners and external agencies to enhance participant services and assist staff in learning about and utilizing community resources.
  • Collaborates with the Director of Supportive Services to gather program participant feedback and propose enhancements to services provided.
  • In coordination with the Director of Supportive Services, works with the Director of Housing and Housing department staff to clarify duties, establish policies as needed, and resolve issues in order to enhance housing retention. Meets regularly with the Director of Housing and the Director of Supportive Services to review vacancies, property concerns, and any other pertinent issues.
  • Follows agency policy in handling participant and staff grievances, complaints, appeals, and requests. Consults with the Director of Supportive Services and/or Human Resources as needed.
  • Participates in monthly Porchlight Management Team (M-Team) meetings.
  • Engages in biweekly supervision meetings with the Director of Supportive Services.
  • Responds to after-hours phone calls and resolves urgent issues as necessary.
  • Performs other duties as assigned by the Director of Supportive Services.
  • Case Management:
  • Schedules and conducts property tours with prospective program participants.
  • Completes intake paperwork with new participants, enters enrollments promptly into the Homeless Management Information System (HMIS).
  • Meets regularly with assigned program participants to offer client-centered services that align with housing first and harm reduction approaches.
