The SED Support Coordinator will be responsible for assessing the needs and strengths of children and youth with serious emotional disturbance (SED) and their families; providing support coordination, advocacy, service linkage, and ongoing care coordination to ensure access to appropriate behavioral health, developmental, educational, medical, and community‑based services. The SED Support Coordinator will provide face‑to‑face support, outreach, and coordination with children, youth, parents/caregivers, schools, healthcare providers, behavioral health providers, social services, and other community partners to promote stability, safety, and improved functioning across home, school, and community settings.

Work is performed under regular supervision.

• Performs intake and ongoing assessments of children and youth with serious emotional disturbance (SED) to identify strengths, needs, risks, barriers to services, and appropriate supports.
• Develops and maintains individualized service plans and support coordination goals in collaboration with the child/youth, parent/caregiver, providers, and other members of the treatment team.
• Provides face‑to‑face support coordination and ongoing monitoring of services to children, youth, and families in home, school, community, and agency settings as appropriate.
• Coordinates, refers, and links children, youth, and families to appropriate behavioral health, developmental, educational, medical, substance use, crisis, social service, and community-based resources.
• Provides advocacy and assistance to children, youth, and families in navigating multiple service systems and accessing appropriate and timely supports.
• Collaborates with parents/caregivers, schools, healthcare providers, behavioral health providers, social services, community organizations, and other professionals to promote continuity of care and address barriers to service delivery.
• Provides support, education, and resources to parents/caregivers regarding behavioral health needs, child and adolescent development, available services, treatment recommendations, and strategies to support the child/youth's functioning across home, school, and community settings.
• Participates in and facilitates team meetings, care coordination meetings, and Intensive Care Coordination (ICC)/High Fidelity Wraparound activities as applicable to the needs of the child and family.
