Job Overview

The core responsibility of this position is to facilitate a safe and secure environment by providing treatment program services individually and in group sessions within each housing unit within the Correctional Facility. This position requires interviewing and assessing inmates to determine risk; reviewing criminal history information and past incarceration records; evaluating the threat level to self or others; recommending housing placement;

reassessing inmates during their incarceration; providing case management services; discharge planning; and overseeing the employment status for those individuals placed in the St. Charles County Department of Corrections.

The Re‑Entry Coordinator works in unison as a supportive sharing team and has the ability to make individual decisions. They are prepared to act as a member of the custody staff to ensure the safety of the public, staff, and inmates by possessing the necessary knowledge and physical abilities to work in the corrections environment.

Responsibilities
  • Assess the needs of inmates, determine the level of care, ensure inmates have access to services in a continuum of care by identifying, referring, and authorizing appropriate services, develop treatment plans, and facilitate inmate treatment as needed.
  • Conduct inmate orientation on facility rules, programs, and expectations; oversee daily housing unit activities to promote accountability, participation in evidence-based programming and successful reentry into the community.
  • Create and update comprehensive treatment plans based on needs assessment information, guide positive decision making, recommend services, track behavior and program participation, and facilitate reentry, cognitive-behavioral, vocational/educational programs as needed.
  • Screen inmates with the LS/CMI Risk Assessment and other risk/need assessments for program placement and perform periodic review/reassessment of inmate status, modifying housing and program recommendations as needed.
  • Coordinate funding and authorize inmate services; facilitate access to housing, employment, and medical resources; review program relocation requests; and respond to court, probation, and agency inquiries to support successful reentry and inmate management.
  • Develop, implement, and conduct unit programming (social and rehabilitative) for offenders on designated caseloads; review inmates’ risk assessments, conduct interviews, determine risk priority to facilitate the delivery of jail-based inmate programming; maintain records, prepare reports, and compose correspondence related to offender participation in facility programs.
  • Evaluate inmate program involvement and refer inmates to additional inmate-based programs as required.
  • Handle informal program complaints at the housing unit level; assist in the administration of offender orientation programs for the unit; serve on various institutional committees concerning assigned caseload; write proposals for housing unit programs.
  • Implement court-ordered services and assist with pre‑trial risk assessments.
  • Maintain regular contact with segregated inmates and coordinate with mental health staff to address concerns and support safe inmate management.
  • Coordinate discharge planning and connect inmates to community-based resources, treatment programs, and other services to reduce recidivism.
  • Attend meetings, maintain files and records, participate in inmate disciplinary misconduct hearings as needed, communicate with internal departments and external social and professional entities on behalf of inmates, develop status reports on inmates for the courts, and conduct client follow‑up at required intervals.
